In an effort to reduce costs associated with record retention, the Town stores many of its records at an off-site facility. It is recommended that when making a public records request, you make your request as detailed as possible and in written form so that we can fulfill it accordingly.

*The cost of copies is as follows:

  • Single-sided: 15¢
  • Double-sided: 20¢ for double sided
  • Certified Copy: $1.00 per page

Please be advised that other charges may apply depending on the nature of the request.

**Note: Protected Health Information

All rules governing the use and disclosure of Protected Health Information (PHI) will be governed by nationally recognized standards, rules, regulations, policies and procedures. Generally, there is limited access to a patient’s Emergency Medical Services (EMS) patient medical record without the express, written consent of the patient or without meeting the specific criteria set by the Federal Register. In order to ensure compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Standards for Privacy of Individually Identifiable Health Information (45 CFR Part 160 and 164), Medical information covered by the patient/­physician privilege may be released to any person who presents a properly authenticated release for the information signed by the patient or legal designee.

The use of this online form to request EMS reports is not permissible and, therefore, you are directed to contact the Fire Rescue Department at 954-797-1213 to assist you in your request.
